Required Coverage Documentation
- Bodily Injury — included (with limits) or rejected
- Medical Payments — included or rejected
- Comprehensive — included (with deductible) or rejected
- Collision — included (with deductible) or rejected
UM/UIM Form Requirement
- Required for ALL vehicle policies
- Applies to: auto, motorcycle, boat, commercial auto, golf cart, ATV, RV/motorhome, and all recreational units
- Must be signed and dated by the insured
- Must clearly document stacked, non-stacked, or rejected selection
Result Codes
- ✓ PASS — All checks passed
- ⚠ WARNING — Non-critical issues (e.g. missing date)
- ✗ FAIL — Critical issues — do NOT bind
